※Only a 10 minute procedure.
※Relatives who live together have to apply if the owner is under sixteen years old.
※The photo must be taken not more than 6 months before the date of submission. The photo size: length 4.5cm width 3.5cm （In the case of under 16 years old, photograph is not necessary.）
